Growing Healthy Worksites Application


Applications for the Growing Healthy Worksites project will be reviewed by the ArCOP board. Positions and funding are limited. 

The following application will be used to gather information on employers interested in participating in the Growing Healthy Worksites project: in using evidence-based tools to implement Policy, System, and Environmental changes at their worksites to create sustainable changes to benefit their employees' health. 

Questions in the application have point values. The point system will be utilized by the ArCOP board to give your organization a preliminary score. Please answer each question honestly as it pertains to your worksite.

1. During the last 12 months, did your worksite conduct an employee needs and interests assessment for planning health promotion activities?
Answer “yes” if, for example, your organization administers focus groups or employee satisfaction surveys to assess your employee health promotion program(s). Answer “no” if your organization administers general surveys that do not assess your employee health promotion program(s).

Answer “yes” if your worksite adheres to a statewide, countywide, or citywide policy banning tobacco use in the workplace.
